Electrocardiogram (EKG)
According to the Centers for Disease Control and Prevention, more than 600,000 people in the United States die from heart disease each year, making it the leading cause of death for both men and women. Noting this, medical professionals often suggest that their aging patients or at-risk patients take an electrocardiogram, or EKG, test to check for signs of heart disease.
EKG tests are non-invasive, painless, safe and fast, and they help our doctors check things like heart rhythm and blood flow as well as diagnose previous heart attacks or anything else that seems abnormal. In the best case scenario, the EKG test confirms good heart health. However, if we see something abnormal, we will likely send you for further testing to get an idea of how to best treat or manage the issue.

We are proud to offer laboratory services with on-site testing using state-of-the-art technology so that you and your doctor can get your results as quickly as possible.
We can offer not only simple diagnostic tests but more complex ones, too. Once you’ve been diagnosed, we can start treatment so you can begin to feel better sooner. We can have certain test results in just a few minutes instead of having to wait to get the results from another laboratory.

Routine Gynecological Care
Here at CNY Family Care we believe in the importance of preventative care and having routine screenings to maintain your overall good health. Early detection can be the key to treating women’s cancers and increasing the chances of a full recovery. Our practice offers various cancer screening options so that you can ensure your optimal health.
Clinical breast exams are conducted during each exam to search for any changes in your breasts or unusual lumps. We will ask about your family history and if any members have been diagnosed with breast cancer. Annual mammograms begin at the recommended age of 40.
It is important that you conduct self-breast exams at home so that you can notice any changes in your breast between visits.
Cervical Cancer Screenings
Pap smears are important for finding any changes in your cervical cells. These tests begin at the age of 21. The HPV test can also be conducted at this time since the HPV virus can lead to cervical cancer. Even if you have been vaccinated against HPV, it is still important that you follow the screening recommendations for your age group.
Colorectal Cancer Screenings
The screening for colon cancer typically begins at the age of 45, unless otherwise recommended by your doctor. This is accomplished by analyzing a stool sample or by examining the colon and rectum (colonoscopy).
Endometrial Cancer Screenings
During menopause, women should report any unexpected vaginal changes such as bleeding or spotting. There are many causes of postmenopausal bleeding that can be benign such as uterine polyps or fibroids. However, uterine cancer, or endometrial cancer, can also cause abnormal changes during menopause. If you notice any changes, please report them to your doctor so that a biopsy can be conducted to determine whether there is a concern for cancer.

Immunizations
CNY Family Care encourages everyone, adults and children alike, to immunize. However, immunizations are the single most important medical measure that a parent can do to keep their child healthy. CNY Family Care adheres strictly to the American Academy of Family Practice immunization guidelines and the Advisory Committee on Immunization Practices for the Centers for Disease Control. Electromyography (EMG) Studies
CNY Family Care offers routine Electromyography (EMG) studies to assess the health of muscles and the nerve cells that control them. EMG results can reveal nerve dysfunction, muscle dysfunction, or problems with nerve-to-muscle signal transmission.
X-Ray
CNY Family Care offers onsite X-rays as a guiding tool to view and assess broken bones, monitor, diagnose joint injuries and infections, arthritis, artery blockages, abdominal pain, and other issues.
Pulmonary Functions Testing
NY Family Care provides Pulmonary function tests (PFTs) as a noninvasive way to see how your lungs are working. The tests measure lung volume, capacity, rates of flow, and gas exchange. This information can help our providers diagnose and treat lung disorders.
Nutrition & Weight Management Services
Nutrition is an essential part of preventive medicine; the right nutrition can help support health and well-being and prevent and treat chronic diseases, including obesity. Here at CNY Family Care, we recognize the essential need for patients to help themselves make dietary corrections to promote healthy eating and lifestyle and address various dietary issues with the guidance of our Registered Dietitians.
Medicare Annual Wellness Visits
Medicare patients benefit from the Annual Wellness Visit that is 100% covered by Medicare. These visits come at no cost to Medicare patients, and CNY Family Care is pleased to offer these “healthy” visits to our patients. The visits include the “Annual Wellness Visit” and the “Welcome to Medicare Visit.” As a Medicare patient, you can only qualify for one of the free “wellnesses” visits this year.
The Welcome to Medicare visit is performed within your first 12 months of enrollment. Your physician will update your current conditions, prescriptions, and medical and family history during the exam. Your doctor will make sure you are on schedule for future tests and screenings. This visit is a one-time exam so take advantage of this offer before your qualification period passes.
WHAT IS THE “ANNUAL WELLNESS VISIT”?
The Annual Wellness Visit is a preventive services screening to help you plan for a healthy future. This annual exam will be performed one year after the Welcome to Medicare visit. You will be assessed on different factors contributing to healthy living during your visit. These assessments include depression screening, fall risk, home safety, and cognitive impairment. Your physician will update your medical and family history, current conditions, prescriptions, other providers, and suppliers.
WHO QUALIFIES FOR THE “ANNUAL WELLNESS VISIT”?
If you have been enrolled in Medicare for over 12 months, you qualify for the Annual Wellness Visit. If you have not received the Welcome to Medicare Visit and have been more than 12 months since your enrollment date, you qualify for the Annual Wellness Visit.
WHAT IF I HAD AN “ANNUAL WELLNESS VISIT” LAST YEAR?
If you had an Annual Wellness Visit last year, you qualify for a subsequent Annual Wellness Visit. You qualify for the subsequent visit one year after your initial Annual Wellness Visit. The subsequent visit includes updates to your screening schedule and your past Annual Wellness Visit.
WHY SHOULD I HAVE A WELCOME TO MEDICARE OR ANNUAL WELLNESS VISIT?
Both visits are 100% covered by Medicare. They also give you the experience of speaking with your physician about your overall health. In most cases, patients visit their doctor when they have a specific ailment. Your physician can focus on your overall health and health plan during your preventive visits for the next 5 to 10 years. Bone Density
CNY Family Care has access to bone density testing (dexascan) on site, a test that measures bone density, a factor in osteoporosis. Osteoporosis is an illness that makes people prone to breaking bones. It is common in post-menopausal women and in some other medical conditions. A bone density measurement is similar to an x-ray and takes from 5-20 minutes. It is simple, safe, painless, and noninvasive. This measurement helps your physician to determine if you need to take certain steps to protect your bone health.
Advanced Imaging Services
CNY Family Care has access to advanced diagnostic imaging, on site, including imaging services in Mammography including Digital Mammography, Stereotactic Breast Biopsy, Fluoroscopy, MRI, including Breast MRI, MRI guided Breast biopsy, CT scanning, Ultrasound, including Ultrasound guided Breast and Thyroid Biopsies and Bone Densitometry.
Chronic Care Management
Chronic Care Management encompasses the oversight and education activities by health care professionals to help patients with chronic diseases and health conditions. Chronic care management helps patients systematically monitor their progress and coordinate with experts to identify and solve any problems they encounter in their treatment. Chronic care management services are offered to our patients who have Medicare. We have partnered with a company called Wellbox to provide these additional care management services to our patients. Medicare has been encouraging participation in this program because it can help catch health concerns and complications before they become problems.
