Thank you for considering CNY Family Care, LLP for your health care needs. We are a Family Oriented Medical practice with Physicians, Physician Assistants,  Nurse Practitioners and  Diabetic Educators. All of our physicians are Board Certified by the American Board of Family Medicine, Academy of Family Physicians. Our Nurse Practitioners and Physician Assistants are individually board certified and licensed by the State of New York. They work directly with our physicians to provide all of our patients with the best family care possible.

We have a Diabetes Care Management Team, that is recognized by the National Committee for Quality Assurance, and the American Diabetes Association for Excellence in Diabetes care and self-management.

One of our main goals at CNY Family Care, LLP is to provide all our patients with continuous and comprehensive health care. As part of this care model, we ask all our patients to select one personal physician. The physician you choose will be your Primary Care Physician. They will maintain an ongoing relationship with you and oversee your care. They will be in charge of the care team of nurses, Nurse Practitioners, Physician Assistants and Diabetic Educators who might interact with you when you visit the office. Our care team will work towards ensuring that you receive the appropriate age-related preventative testing and treatment as well as the latest evidence-based treatment (proven and documented) for your individual medical condition(s).

We do not accept Workers Compensation Insurance AND we are currently only accepting new patients who are infants and adolescent patients and young adult patients transitioning from a pediatrician’s office. We are not accepting new adult patients currently.

We are available to our patients and their families when they need us. We partner with our patients to provide a proactive approach to medical care, which improves their quality of life.

Quick & Easy Check-In:

Our self-check-in kiosks are located in suite 101 (our main suite) on the first floor to expedite your check-in experience. Kiosks are available for adult patients. Minors will need to check in with their parents or guardian at the front desk.

Appointment Pre-Registration:

Our Patient Pre-Check-In process offers a way for our patients to complete parts of the check-in from a personal device before arriving at the office, minimizing time spent at the front desk. The secure pre-check-in link is sent to you via text or email and works on various devices, including smartphones and tablets.

Extended Hours:

We feel that access to your primary care physician is one of the most important aspects of good healthcare. In an effort to accommodate as many of our patients as possible, we offer extended office hours during the work week and also on the weekends. Our extended office hours are Monday – Friday 7:30 am – 9:00 pm.

Extended Hours Appointment Fee:

Please note that there is an additional $25.00 fee for all appointments starting at 5pm during the week, and ALL Saturday and Sunday appointments. While this fee is often covered by insurance plans, patients should check with their insurance company to see if their insurance covers this fee. Patients will be responsible for this fee if it is not covered by their insurance.
