ANNOUNCEMENT:

We want to remind all of our patients to BRING IN YOUR INSURANCE CARD WHEN YOU COME FOR YOUR FIRST OFFICE VISIT OF 2013. Our front desk staff has been asked to confirm all patient's contact information and scan insurance cards for each patient. It is important for billing purposes that we have all your personal information updated in our system to prevent you from being billed for claims that your insurance company should be covering for you. Also, please be prepared to pay your co-pay at the time of service. We appreciate your patience as we update our records for 2013.

A Family Medical Practice

Thank you for considering us for your health care needs. The purpose of this Website is to tell you a little about our practice, in hopes that we can make each visit as successful as possible.

It is our policy to participate with the major insurance companies. We participate with most local HMOs as well as PPOs and other managed care programs.

We are currently accepting new patients.

From Route 481, take exit 7 (from north or south) then take a right onto Collamer Road (Route 298) and then a left onto Brittonfield Parkway. We are the last building on the right.

Office Hours

Regular Office Hours:

Monday through Friday: 7:00 a.m. – 9:00 p.m.
Saturday: 9:00 a.m. – 2:00 p.m.
Sunday: 10:00 a.m. – 2:00 p.m.

Extended Hours:

We have developed extended hours in order to accommodate our patients. We feel access to your primary care physician is one of the most important aspects of good health care. Because of our extended hours we feel the need to visit community "Urgent Care Centers" should be minimal. Please contact our office before going to an Urgent Care Center. If you visit an Urgent Care Center without notifying our offices, it may not be covered by your insurance.

Insurance Accepted

CNY Family Care participates with most insurances, however it is the patient's responsibility to know what is and is not covered by their insurance plan.
